DHS Piano Tutoring: Registration

Student Registration:

Student/parent applicants must fill out the following:

1. DHSPTP Student Application Form

2. DHSPTP Waiver of Liability (included in the application form)

Students/parents must submit all applicable forms (included in the application form) to the DHS Piano Tutoring Program team through email.

Tutor Registration:

Tutor applicants must fill out the following:

1. DHSPTP Tutor Application Form

2. DHSPTP  Tutor Contract (included in the application form)

3. DHSPTP Waiver of Liability (included in the application form)

Tutors must submit all applicable forms (included in the application form) to the DHS Piano Tutoring team through email.
